I spent some time this afternoon looking at and testing a few different free online contact forms.
My favorite at the end of the day: Wufoo
Because it is intuitive and easy to use, and it works.
Here's a couple tips for anyone who'd like to try it.
To create a form, click any Field name in the left hand panel. The field will display in the right hand column. Then clicking anywhere in the field in the right column, will bring up the Field Settings tab.

In the Field Settings tab, you can edit the Field Label, and display options.

From the Form Manager, click on Code to grab the code to copy and paste on your web page or blog.

Select and copy the code.

Back at the Form manager, click on Notifications, to set the email address to receive inquiries.
